倍频532nm激光治疗糖尿病视网膜病变的效果评价 被引量:4

摘要 目的 观察倍频532 nm激光对糖尿病视网膜病变的治疗效果.方法 172例(298只眼)糖尿病患者通过眼底照相或眼底荧光造影检查诊断为糖尿病视网膜病变,根据病变分期进行局限性光凝、次全视网膜光凝以及全视网膜光凝,合并黄斑水肿者进行格栅样光凝,观察激光治疗前后患者视力,眼底出血、渗出以及新生血管情况.结果 随访3~12个月,增殖前期病变患者中,213只眼视网膜出血、渗出明显吸收,17只眼进展为增殖期;增殖期患者中,27只眼新生血管萎缩,10只眼因出现严重病变接受玻璃体手术治疗;弥漫性黄斑水肿患者中,29只眼水肿明显吸收,17只眼囊样水肿部分吸收,28只眼有临床意义的黄斑水肿基本吸收.结论 倍频532 nm激光治疗糖尿病视网膜病变具有明确疗效,治疗时机对于控制病情的发展具有重要意义. Objective To observe therapeutic effects of diabetic retinopathy with frequency doubling laser photocoagulation of 532 nm. Methods 170 cases (298 eyes) with diabetic retinopathy diagnosed by fundus photography or fundus fluorescein angiography,the laser photocoagulation includes limitations photocoagnlation,subtotal retinal photocoagulation,panretinal photocoagulation and grid photocoagulation by the stage of the diabetic retinopathy.Visual acuity,fundus hemorrhage,exudation and neovascularization were observed. Results Followed up 3-12 months,among the patients with preproliferative diabetic retinopathy,the retina hemorrhage/exudation of 213 eyes were apparently absorbed,17 eyes evolved proliferating phase.Among the patients with evolved proliferating phase,the new blood vessels of 27 eyes were atrophied,10 eyes were implemented vitrectomy because of the severe lesions.The edema of 29 eyes were appar- ently absorbed among the diffuse macular edema patients,and the cystoid macular edema of 17 eyes were partly ab- sorbed,the clinical significant macular edemas of 28 eyes were basically disappeared. Conclusion The frequency doubling laser photocoagulation of 532 nm is a effect methods in treatment of diabetic retinopathy,and the treatment opportunity is of great significant to control the condition of development
