正位回肠膀胱替代术的术后护理

目的探讨膀胱全切、回肠膀胱替代术的术后护理。方法从1998—2007年行标准根治性膀胱全切术及正位回肠膀胱替代术患者中选取获得完整随访的膀胱癌患者47例,对其术后护理过程进行回顾性分析。结果 47例患者术后均恢复良好,无围术期死亡及伤口感染,手术并发症发生率14.9%(7/47),尿动力学指标均有较好的恢复。结论回肠膀胱替代术后正确的护理是降低围术期死亡率、减少手术并发症的关键因素。

Objective To explore the postoperative nursing of cystectomy and orthotopic ileal neobladder reconstruc- tion. Methods 47 cases of bladder cancer with complete follow-up were selected from the patients with standard radical cystec- tomy and ortbotopic ileal neobladder reconstruction from 1998 to 2007. Retrospective analysis was conducted on the postoperative nursing proceed. Results All the 47 patients was with good recovery after operation without mortality during preoperative period or wound infection. Incidence rate of the operative complications was 14.9%（7/47）, urodynamics indexes were all recovered bet- ter. Conclnsion Proper nursing after orthotopic ileal neobladder reconstruction is critical factors to decrease mortality rate in preoperative period and reduce the operative complications.
