2型糖尿病足的临床特点及相关因素分析

Analysis on the Clinical Characteristics and Related Factors of Type 2 Diabetic Foot

目的探讨2型糖尿病足的临床特点和其相关因素。方法选取我院2011年1月至2012年12月收治的196例2型糖尿病患者,根据有无糖尿病足溃疡分为观察组(92例)和对照组(104例),观察组有糖尿病足,对照组无糖尿病足;比较两组患者的临床特点,并通过多元线性回归方程分析糖尿病足相关因素。结果观察组吸烟史、糖尿病病程、MAP、FBG、PBG、TC、TG和HbA1c显著高于对照组,eGFR显著低于对照组,差异均具有统计学意义(均P<0.05);多因素回归分析显示与糖尿病足显著相关的因素有:吸烟、病程≥10年、周围神经病变、eGFR低和外周血管病变,差异均具有统计学意义(均P<0.05)。结论吸烟、病程≥10年、周围神经病变、eGFR低和外周血管病变是糖尿病足的主要相关因素。

Objective To investigate the clinical characteristics of type 2 diabetic foot and its related factors. Methods 196 patients with type 2 diabetes mellitus patients were enrolled in our hospital from January 2011 to December 2012. According to the presence or absence of diabetic foot ulcer, the cases were divided into observation group （92 cases） and control group （104 cases）. The observation group were with diabetic foot, and the control group were without diabetic foot. The clinical characteristics of two groups of patients were compared, and the related factors to diabetic foot were analyzed through multiple linear regression. Results In the observation group, smoking history, duration of diabetes, MAP, FBG, PBG, TC, TG and HbAlc were significantly higher than those of the control group, but eGFR was significantly lower than that of the control group, and the difference was statistically significant （P 〈0.05）; multi factor regression analysis showed that the factors that significantly associated with diabetic foot were： smoking, duration≥ 10 years, peripheral neuropathy, low eGFR and peripheral vascular disease; the differences were statistically significant （P 〈0.05）. Conclusions Smoking, history of 10 years or longer, peripheral neuropathy, low eGFR and peripheral vascular disease are the main related factors to diabetic foot.
