医患双方对双向转诊认知度现况调查

To Investigate the Current Status of Awareness for Two-way Referral of Doctors and Patients

目的了解复兴医院与月坛社区卫生服务中心的医生以及就诊患者对双向转诊的认知度,发现问题并提出建议,为我国进一步完善双向转诊制度提供参考依据。方法采取问卷调查的方法,随机抽取首都医科大学附属复兴医院医生75名,月坛地区10个社区卫生服务站的医生44名,医院患者166名,社区患者103名,对资料进行统计描述分析。结果 (1)住院患者中71.69%表示出院后出现不适时首选到大型医院就诊,社区患者在出现不适时73.79%首选到社区中心就诊;(2)社区医生上转病人的比例高于住院医生下转病人,社区医生对转诊流程的知晓程度高于医院医生,有显著性差异(P<0.01);(3)患者中知道医院与社区开展双向转诊工作者占44.24%,13.75%了解具体流程。对双向转诊的接受程度,社区患者与住院患者差异存在统计学意义(P<0.01)。结论目前双向转诊的工作仍存在困难与不足,医院医生及患者对现行双向转诊的认知度有待提高。

OBJECTIVES To investigate the current status of awareness for two-way referral of doctors and patients in Fuxing Hospital and Yuetan community health service institutions, find out problems and put forward some suggestions so as to provide a reference for further improvement of two-way referral system in China. METHODS There were 75 doctors and 166 inpatients of Fuxing Hospital, 44 doctors and 103 outpatients of Yuetan community health service institutions randomly enrolled. All subjects answered the questionaire of two-way referral. Data were collected and analyzed statistically. RESULTS （1）A total of 71.69% of the inpatients preferred hospital service when in need of visiting a doctor, whereas 73.79% of the outpatients choose community service institions. （2）Up-referral of doctors in community health institutions were more than down-refferral of doctors in hosptial. The awareness of procedure of referral was significant different between doctors in community service institions and in hospitals（P〈0.01）. （3）There were 44.24% of all patients knew two-way referral and 13.75% knew the procedure. The acception of two-way referral was significant different between patients in community service institions and in hospitals（P〈0.01）. CONCLUSION There are some problems of current two-way referral system, the awareness of doctors of hospital and all patients need to be improved.
