摘要
目的分析评价家庭医生团队签约服务对高血压和糖尿病患者的健康管理效果,为进一步优化相关政策提供科学依据。方法设计问卷,调查签约和未签约患者的健康管理情况和健康管理效果,并进行对比分析。结果家庭医生团队签约慢性病服务初步显效,签约患者的健康意识和依从性提高,血压及血糖情况得到改善,治疗费用下降,但健康管理服务质量和效果还有提升空间。结论加强家庭医生的宣传工作,引导居民积极签约;完善家庭医生团队建设,规范家庭医生的工作,加强对慢性病患者的健康管理。
OBJECTIVE To analyze and evaluate the effect of family doctor group service for chronic diseases health management, and provide scientific references for optimizing relevant policies. METHODS Researching the health management condition and effect of chronic patients by questionnaire investigation. RESULTS Family doctor contracting service for chronic diseases health management has obtained achievements, the health awareness and disease control of signatories improved, and expenditures reduced, while the quality and effect still need to be improved. CONCLUSION Strengthening the propaganda of family doctor and guiding residents signing; improving team construction and regularizing the work of family doctor to improve the management of patients with chronic diseases.
