右旋美托咪定对中老年患者单肺通气术后早期认知功能改变的影响 被引量:5

摘要 我们将右旋美托咪定(DEX)应用于单肺通气(OLV)中老年患者围术期,探讨其对患者术后认知功能障碍(POCD)发生率的影响.一、资料与方法1.一般资料:本研究已获本院医学伦理委员会批准,并与患者签署知情同意书.择期行OLV的中老年患者60例,年龄(60±9)岁,体质量指数(21.5±3.8) kg/m2,美国麻醉医师协会(ASA)分级和纽约心脏病协会(NYHA)分级均为Ⅱ~Ⅲ级,简明精神状态量表(MMSE)评分≥26分.将患者随机分为DEX组(n=30)和生理盐水对照组(NC组,n =30).2.方法:患者常规监测心率(HR)和有创平均动脉压(MAP).静脉注射咪达唑仑0.01 ~0.02 mg/kg、舒芬太尼0.4~0.6 μg/kg、罗库溴铵0.6~1.0 mg/kg和依托咪酯0.3 mg/kg麻醉诱导,待麻醉诱导满意后行双腔气管内插管并行机械通气,潮气量:双肺通气时10 ml/kg,OLV时8 ml/kg,通气频率10~15次/分,吸呼比=1∶2,吸入氧浓度100%,维持PET-CO2在 35 ~40 mm Hg.
