连续多普勒测量肺动脉压力在基层医院的应用

The application of continuous doppler measurement of pulmonary artery pressure in primary hospital

摘要 目的 探讨在基层医院中应用超声心动图测量肺动脉压力的诊断价值.方法 回顾性分析60例经超声心动图检查诊断为肺动脉高压患者的病例资料,包括心脏二维及多普勒声像图.结果 58例患者超声心动图表现为不同程度的右心房室增大,右室壁肥厚,肺动脉内径增宽,并伴有三尖瓣反流,应用瓣膜反流法估测肺动脉压力;2例先天性心脏病患者(室间隔缺损)应用分流法估测肺动脉压力.结论 超声心动图测量肺动脉压力操作简便,无创安全,可重复性好,值得在基层医院中大力推广. Objective To investigate the diagnostic value of echocardiographic measurement of pulmonary artery pressure in primary hospital.Methods The clinical data of 60 patients with pulmonary hypertension diagnosed by echocardiography were retrospectively analyzed,including heart two-dimensional and doppler ultrasonography.Results Fifty-eight patients' echocardiographic showed right atrial and ventricle enlargement in different degrees,right ventricular hypertrophy,pulmonary artery diameter broadening,accompanied by tricuspid regurgitation,application of valvular regurgitation to estimate the pulmonary artery pressure; shunt method was used to estimate the pulmonary artery pressure for two patients with congenital heart disease (ventricular septal defect).Conclusions Echocardiogram measurement of pulmonary artery pressure is simple,noninvasive and reproducible,and it is worth promoting vigorously in primary hospital.
