摘要
目的探讨宫颈癌中人乳头状瘤病毒(HPV)16型感染和P53基因表达情况,并探讨两者之间的交互作用。方法收集119例宫颈鳞癌患者为病例组,124例经检查证实无宫颈癌病变的患者为对照组;采用PCR及免疫组化法检测HPV16DNA及P53基因的表达情况。结果HPV16型感染阳性率病例组88.7%,对照组5.2%,两组比较差异有统计学意义(X2=173.4,P〈0.05),P53基因表达阳性率病例组67.4%、对照组11.3%,两组比较差异有统计学意义(X2=80.1,P〈0.05);多元回归模型结果显示,HPV16型感染及P53基因表达呈现阳性均显著增加宫颈癌的发病风险,OR为74.1及16.2、95%CI:30.8~181.5及7.9~34.1(P〈0.05);由Spearman相关分析可知,HPV16型感染和P53基因表达相关(P〈0.05)。结论HPV、P53是宫颈癌独立的致癌物,两者在宫颈癌的发展过程中有交互作用。
OBJECTIVE To explore the incidence of human papillomavirus 16 (HPV16) infections in the cervical cancer patients, discuss the expression of P53 gene, and observe the interaction between the two. METHODS A total of 119 patients with cervical squamous carcinoma were selected as the case group, while 124 patients without confirmed cervical lesions were chosen as the control group, then the HPV 16 DNA was detected with the use of PCR, and the expression of P53 gene was observed through immunohistochemistry. RESULTS The positive rate of HPV16 infection was 88.7% in the case group, 5.2% in the control group, the difference between the two groups was statistically significant(X2=173.4,P〈0.05); the positive rate of the expression of P53 gene was 67.4% in the case group, 11.3V0 in the control group, the difference between the two groups was statistically significant (X2= 80.1, P〈0.05). The result of multivariate regression model indicated that the HPV16 infections and the expression of P53 gene would significantly increase the risk of cervical cancer, with the OR value of 74.1, 16.2, the 95% CI varying between 30.8 and 181.5 or between 7.9 and 34.1 (P〈0.05). The Spearman analysis showed that there was interaction between the HPV16 infection and the expression of P53 gene (P 〈 0. 05 ). CONCLUSION The HPV infection and the expression of P53 gene are the independent risk factors for the cervical cancer, both play an interaction role in the pathogenesis of cervical cancer.
