小儿脑性瘫痪危险因素的1∶2病例对照研究

A 1∶2 MATCHED CASE-CONTROL STUDY OF THE RISK FACTORS FOR CEREBRAL PALSY IN CHILDREN.

为探索小儿脑瘫的危险因素 ,对 70例脑瘫患儿及 140例对照进行以医院为基础的 1∶ 2配比病例对照研究 ,数据处理采用条件 L ogistic回归分析。单因素分析提示既往不良产史、早产、双胎妊娠、出生体重 <2 5 0 0 g、延迟哭≥ 5分钟、妊娠间隔时间≤ 12月、孕 6～ 9月阴道流血及黄疸与小儿脑瘫的发生有关联。经多因素分析 ,结果选入模型的有妊娠间隔时间短、孕 6～ 9月阴道流血、低出生体重、双胎妊娠、延迟哭及黄疸。提示脑瘫的发生可能是多因素作用的结果 ,尚待进一步的研究。

To study the risk factors of cerebral palsy(CP),a hospital based 1∶2 matched case control study was carried out in 70 children with CP and 140 controls individually matched for sex and age,were selected for each index case.The data were analyzed by conditional logistic regression.Nine risk factors were suggested from univariate analysis,namely poor previous obstetric history,gestation<37wk,twins,birth weight below 2500g,time to cry≥5minutes,short pregnancy intervals,third trimester bleeding and icterus.The risk factors confirmed by multivariate analysis were short pregnancy intervals,third trimester bleeding,low birth weight,twins,delayed crying and icterus.CP maybe related to multiple risk factors.The contribution of the adverse factors in the origin of CP needs an intensive study.
