认知障碍程度对阿尔茨海默病患者脑脊液各类炎性细胞因子表达影响 被引量:5

摘要 目的:观察认知障碍程度对阿尔茨海默病(AD)患者脑脊液(CSF)各类炎性细胞因子表达影响。方法:连续选择近期在我院住院的AD患者39例,对照组选择同期参加体检的健康中、老年人25例,入选对象接受了CSF促炎性细胞因子(IL-1β、IL-6和TNF-α)及抗炎性细胞因子(IL-4、IL-10和TGF-β1)检测,还对AD患者进行了"AD评定量表认知部分"(ADAS-Cog)评估。结果:AD组CSF促炎性细胞因子IL-1β、IL-6及TNF-α表达水平均显著高于对照组,而抗炎性细胞因子IL-10及TGF-β1表达水平明显低于对照组(P均<0.05~0.01);轻度认知损害组(ADAS-Cog总分<20分,22例)CSF促炎性细胞因子IL-1β、IL-6及TNF-α表达水平均显著低于中、重度认知损害重组(ADAS-Cog总分≥20分,17例),但明显高于对照组(P均<0.05~0.01);轻度认知损害轻组CSF抗炎性细胞因子IL-4、IL-10及TGF-β1表达水平均显著高于中、重度认知损害重组,但明显低于对照组(P均<0.05~0.01)。结论:AD患者存在明确的CSF各类炎性细胞因子表达异常,且随着认知损伤严重而进一步活跃。 Objective: To explore the effect of cognitive disorder at different degree on the expression of inflam- matory cytokines in the cerebrospinal fluid (CSF) of Alzheimer~ patients (AD). Methods:Thirty- nine patients with AD hospitalized in our hospital recently were selected consecutively while 25 healthy middle - aged and aged who participated in physical examination at the same time were selected as controls. Proinflammatory cytokines (IL - 1 [3, IL - 6 and TNF - a) and anti - inflammatory cytokines ( IL - 4, IL - 10 and TGF - [31 ) in CSF of all par- ticipams were examined, while patients with AD were also evaluated by Alzheimer' s Disease Assessment Scale - cognitive subscale( ADAS - Cog). Results: The concentrations of IL - 1 [3, IL - 6 and TNF - ot in CSF were signifi- cantly higher in AD group than those in control group, but the concentrations of IL - 10 and TGF - [31 in CSF were lower in AD group than those in control group(P 〈0.05 or P 〈0.01 ). The concentrations of IL- 113, IL-6 and TNF -or in CSF were significantly lower in the mild group( ADAS -Cog 〈 20,22eases) than those in the moderate and severe group of cognitive disorders (ADAS - Cog I〉20,17cases) but were higher than those in the control group (P 〈0.05 or P 〈0.01 ). The concentrations of IL -4, IL - 10 and TGF -/31 in CSF were significantly higher in the mild group than those in the moderate and severe group of cognitive disorders but were lower than those in the control group( P 〈0.05 or P 〈0.01 ). Conclusion: It is eertain that inflammatory cytokines are expressed abnor- mally in CSF of patients with AD and would vary with the degree of cognitive disorders.
