腘肌及腘肌腱损伤的MR诊断 被引量:1

摘要 目的回顾性分析腘肌及腘肌腱损伤的MR表现,了解其影像学特征,提高诊断和鉴别诊断水平,减少误诊率。方法患者共26例,均行膝关节外旋15°的矢状位快速自旋回波T1WI序列、矢状位脂肪抑制STIR序列和冠状位自旋回波T2WI及质子相双回波序列扫描。结果 26例患者中,单纯腘肌损伤为11例,腘肌腱损伤5例,腘肌及腘肌腱同时损伤者为10例。腘肌及腘肌腱的损伤,以矢状位脂肪抑制STIR序列及冠状位自旋回波T2WI序列显示病变最为清楚,可显示腘肌急慢性损伤水肿所致的长T2高信号,同时可显示腘肌腱撕裂所致的连续性中断。轴位快速自旋回波T2加权序列也可清楚显示腘肌损伤所致信号异常。结论 MR成像是显示腘肌及腘肌腱损伤最佳的影像学手段,通过常规方位扫描图像可做出准确的诊断。 Objective To retrospectively analyze the MR performanle of popliteus muscle and the popliteus tendon inju- ries and to understand its imaging features so as to improve the diagnosis and differential diagnosis and to reduce the rate of misdiagnosis. Methods A total of 26 cases underwent knee external rotation 15 ~ sagittal fast spin-echo T1WI sequence, sagittal fat-suppressed STIR sequence, coronal spin echo T2WI and proton phase dual-echo sequence scan. Results In 26 patients, the popliteus muscle injuries were present in 11 eases, the popliteus tendon injury in five cases, the popliteus muscle and tendon injuries in 10 cases. Popliteus muscle and tendon injuries were shown most clearly in sagittal fat sup- pression STIR sequence and coronal spin echo T2WI sequence, which displayed the popliteus muscle acute and chronic in- jury edema due to long Tz signal, while, the popliteus tendon tear caused by discontinuity. The signal abnormalities can al so be clearly demonstrated in axial fast spin-echo T2-weighted sequence. Conclusion MR imaging is the best means of im- aging of the popliteal muscle and tendon injuries, and an accurate diagnosis can be made by conventional scan image.
