摘要
为评价间日疟原虫 PCR检测试剂盒在疟疾现场监测中的效果 ,采用该试剂盒和常规镜检法对安徽省疟疾流行区的 940份血样进行了检测 ,结果显示 ,该 PCR检测试剂盒对 2 34例在校中学生的人群带虫调查的阳性检出例数 (5例 )高于常规镜检 (2例 ) ,所需检测时间和费用均低于常规镜检。同时 ,采用两法对 5 19例门诊发热病人血片 (镜检初检阳性 16例 ,阴性 5 0 3例 )进行复核 ,镜检复核发现漏诊 2例 ,PCR复核发现漏诊 6例 ,所需复核时间和费用也均低于常规镜检。采用两法对 187例门诊“三热”病人进行被动病例检测 ,PCR检出阳性例数略高于镜检 ,但单位检测时间和费用也高于镜检。现场评价结果提示 ,该 PCR检测试剂盒可替代镜检用于疟疾监测中大样本检测 ,且具有敏感、特异、高效和价廉的特点。
To evaluate the effectiveness of malaria surveillance using a new Plasmodium vivax PCR detection kit in the field, 940 blood samples collected from malaria endemic areas of Anhui Province were detected by the methods of both PCR and microscopy respectively.The results showed that 234 blood samples collected from middle school students in Feidong County were detected with both the methods respectively for active case investigation, five positive cases have been detected by PCR while only 2 positive cases have been found by microscopic examination, the detecting time and cost required of PCR were lower than that of microscopic examination. For quality control of primary clinical blood smear examination, 519 clinical fever cases have been determined by microscopy in the hospitals of Qiaotou and Hongmiao townships in Mingguang County and been re checked by both the methods in the central laboratory respectively. 16 positive cases have determined in primary microscopic examination and 18 and 22 positive cases have been found by microscopically re detected and PCR respectively. The re detected times as well as cost required for PCR were also lower than that for microscopic examination. 187 suspected clinical fever cases from the hospitals of Beilong Township and Feidong County have been examined by both methods for the passive cases detection. The positive cases found by PCR were slightly higher than that by microscopic examination. However, the detecting time and cost per case required of PCR were more than that of microscopic examination.The results suggested that this Plasmodium vivax PCR detection kit is sensitive, specific, efficient and economic in large scale determination and could be used in malaria surveillance instead of microscopic examination.
