腔内激光联合高位结扎治疗大隐静脉曲张患者的效果观察

Effect clinical observation of endovenous laser combining with high ligation in treatment of great saphenous varicosity

目的 探讨腔内激光联合高位结扎治疗大隐静脉曲张的临床效果.方法 选择2012年1月～2013年1月在冀中能源邢台矿业集团有限责任公司总医院住院治疗的大隐静脉曲张患者100例为研究对象,根据治疗方法不同分为A组和B组,每组各50例.A组行腔内激光联合高位结扎治疗,B组行传统手术.比较两组患者治疗后总有效率,两组患者术中单侧肢体手术时间、住院时间、术中失血量、两组患者术后皮下淤血、小腿局部麻、小腿局部麻木感、切口感染、浅静脉炎并发症发生率.结果 A组治疗后总有效率为96.0％,B组治疗后的总有效率为76.0％,两组疗效比较,差异有统计学意义（x2=7.356,P＜ 0.05）.A组患者单侧肢体手术时间[（34.12±15.34）min]、住院时间[（3.1±1.2）d]均短于B组[（55.22±11.86）min、（5.3±2.8）d],组间比较差异有统计学意义（P＜0.05）.A组术中失血量[（15.5±2.3）mL]明显少于B组[（53.7±10.7）mL],组间比较差异有统计学意义（P＜ 0.05）.A组术后并发症发生率仅8.0％,B组术后并发症发生率为30.0％,两组并发症发生率比较,差异有统计学意义（P＜0.05）.结论 腔内激光联合高位接扎治疗大隐静脉曲张效果确切,易于操作,具有微创、安全性好、并发症少、缩短住院时间等优点,适合广大基层单位广泛推广和应用.

Objective To investigate the clinical effect of endovenous laser combining with vein ligation in treatment of great saphenous varicosity.Methods From January 2012 to January 2013 in Jizhong Energy Xingtai Mining Group Co.,Ltd.General Hospital,100 patients with saphenous varicosity according to different treatment methods were selected and divided into group A and group B with 50 patients in each group.Group A was given endovascular laser combining high ligation,group B was treated with conventional surgery.Total efficiency rate,undergoing unilateral limb surgery time of the operation,hospital stay,intraoperative blood loss,postoperative subcutaneous congestion,local anesthetic calf,calf localized numbness,infection wound,superficial phlebitis incidence of complication in group A and group B were compared.Results The total effective rate of the group A was 96.0%,the total effective rate of B group was 76.0%,the difference was statistically significant （X2 =7.356,P 〈 0.05）.Unilateral limb surgery time and hospital stay in group A [（34.12± 15.34） min,（3.1 ± 1.2） d] were shorter than those in group B [（55.22±11.86） min,（5.3±2.8） d],the differences were statistically significant （P 〈 0.05）.Intraoperative blood loss in group A [（15.5±2.3） mL] was less than that in group B [（53.7±10.7） mL],the difference was statistically significant （P 〈0.05）.Postoperative complication rate in group A （8.0%） was lower than that in group B （30.0%）,the difference was statistically significant （P 〈 0.05）.Conclusion Endovascular laser combining with high ligation in treatment of great saphenous varicosity is easy to operate,it is minimally invasive,safe and good,has less complications,shorter hospital stay,and so on.It is suitable for the majority of grassroots units and promotion,application.
