睾丸扭转26例临床分析

Clinical Diagnosis and Treatment of 26 Cases of Testicular Torsion

目的探讨睾丸扭转的临床诊治水平。方法回顾性分析26例睾丸扭转患者的临床资料,所有患者均以一侧阴囊疼痛为主诉,术前均行睾丸鞘膜穿刺液化验及阴囊彩超检查,穿刺液细胞计数均提示红细胞明显增多,白细胞无增加,多普勒彩超表现为患侧睾丸肿大,睾丸血流消失23例,减少3例。结果 26例患者中患侧复位1例,患侧复位并对侧固定4例,仅行患侧切除11例,患侧切除并对侧固定10例。结论睾丸鞘膜穿刺液化验及多普勒超声检查是一种快速、简便和可重复的临床检测方法。尽早手术探查可提供准确的诊断和治疗,降低睾丸切除风险。

Objective To investigate the diagnosis and treatment of testicular torsion. Methods The clinical data of 26 cases of testicular torsions were summarized and analyzed. All the patients had the essential clinical symptoms of scrotal pain. Puncture liquefaction test showed that erythrocyte increased obviously and leukocyte did not increase. Color Doppler sonography showed one side testicle swelling, 23 cases with decreased perfusion, and 3 cases with no perfusion. Results Among the 26 cases, there was 1 case with orchidopexy, 4 eases with bilateral orchidopexy, 11 cases with orchiectomy, a- mong which 10 cases were with orchiectomy and contralateral orchidopexy. Conclusion Puncture liquefaction test com- bined with color Doppler sonography is the fast, convenient and repetitive clinical method that is of great help to get exact and early diagnosis of testicular torsion. Early surgical exploration can offer an accurate diagnosis and treatment, minimi- zing the risk of testicular orchicctomy.
