超声诊断子宫内膜病变的临床价值分析

Clinical value analysis of ultrasound in the diagnosis of endometrial lesions

目的探讨经阴道超声诊断子宫内膜病变的临床价值。方法选择本院妇产科经病理诊断确诊的80例子宫内膜病变患者作为研究对象,并将经阴道超声检查结果与病理诊断结果进行对比分析。结果术后病理诊断为子宫内膜息肉、子宫内膜增生、子宫内膜癌及子宫黏膜下肌瘤的患者数分别为20、12、22、26例,超声诊断分别为18、8、18、23例,超声诊断的总符合率为83.75%(67/80);经阴道超声诊断的49例子宫内膜良性病变(包括子宫内膜息肉、子宫内膜增生、子宫黏膜下肌瘤)血流显示例数为14例,显示率为28.57%(14/49),动脉阻力指数(RI)=0.67±0.13;子宫内膜癌血流显示例数为18例,显示率为100.00%(18/18),RI=0.38±0.08,两种血流显示率及RI比较,差异均有统计学意义(P<0.05)。结论经阴道超声诊断子宫内膜病变准确率高,且能敏感地显示病变周围及内部的彩色血流信号及其血流动力学变化情况,有利于子宫良恶性病变的鉴别。

Objective To investigate the clinical value of transvaginal ultrasound in the diagnosis of endometrial lesions. Methods 80 cases of endometrial lesions confirmed with pathological diagnosis in the gynaecology and obstetrics of our hospital were chosen as research subject.Transvaginal ultrasound results were analyzed and compared with those of pathological diagnosis. Results The number of patients diagnosed as endometrial polyps,endometrial hyperplasia,endometrial cancer and uterine fibroids submucosal by pathological diagnosis were 20,12,22 and 26 cases respectively, while there were 18,8,18 and 23 cases with ultrasound diagnosis, thus the total correspondence rate of ultrasound diagnosis was 83.75%（67/80）;14 of the 49 cases of endometrial benign lesions（included endometrial polyps, endometrial hyperplasia,submucosal uterine fibroids） detected with vaginal ultrasound diagnosis were detected with blood flow, with a detection rate of 28.57% （14/49）and arterial resistance index（RI）=0.67±0.13;18 cases endometrial cancer were detected with blood flow, with a detection rate of 100.00% （18/18）and RI=0.38±0.08. The diferrences of blood flow detection rate and RI between the two groups were all statistically significant （P〈0.05）. Conclusion Transvaginal ultrasound diagnosis of endometrial disease had high accuracy, and can be displayed with sensitivity around and inside the lesion color flow signals and hemodynamic changes,thus it is conducive in identifying benign and malignant lesions of the uterus.
