关节假体感染的诊断及诊断技术的发展现状 被引量:4

摘要 人工关节置换术主要包括髋关节置换术和膝关节置换术,是目前骨科临床最常见的手术,假体周围感染(prosthetic joint infection,PJI)是其主要并发症之一[1].虽然医生在感染的预防方面已经做了大量的工作,如提高手术标准、降低术中污染、预防性应用抗生素和隔离患者等,但PJI的发生率并没有显著降低[2].据报道,对于初次关节置换术,髋和肩关节置换术的术后感染率为1%,膝关节置换术的术后感染率为2%,肘关节置换术的术后感染率为9%,而对于关节翻修术,其感染率甚至高达40%[3];另外,引起全踝关节置换失败的并发症33%是感染[4].PJI最大的问题是早期诊断困难,临床上一直缺乏准确可靠的早期诊断手段,当出现临床感染症状时多为晚期,常延误治疗并导致手术的失败和关节功能的丧失[5-6].
