Forensic pathological analysis of post-traumatic fat embolism

目的 讨论外伤后肺脂肪栓塞的检验方法、诊断分级标准和死因分析注意事项.方法 收集2008-2011年检验的11例外伤后肺脂肪栓塞的死亡案例,总结死者损伤情况,进行巨检、苏木精-伊红（H-E）染色和特殊染色等病理学检查并得出死因.结果 11例案件各项病理学检查均有不同程度的脂肪栓塞的阳性发现,死因6例为脂肪栓塞,1例为创伤性休克,4例为联合死因.结论 存在脂肪栓塞的案例需依据尸检及组织病理学检查结果并结合临床表现、死亡过程等综合分析,科学判断.

Objective To discuss forensic examination steps and grading criteria of post - traumatic fat embolism and to perform cause - of - death analysis. Methods 11 death cases with post - traumatic fat embolism from 2008 to 2012 were collected to perform injmT evaluation. Then, gross examination, H -E and special staining were conducted to obtain cause - of - death. Results Various degrees of fat embolism were examined in the 11 cases through pathological examinations. As to cause - of - death, 6 cases were due to fat embolism, 1 traumatic shock and 4 combined causes. Conclusion For the cases with fat embolism, comprehensive analysis on autopsy and histological findings, clinical man- ifestations and death course are essential to yield a correct cause -of- death.
