摘要
目的:探讨优质护理服务中使用"一指明"对意识清醒患者行有创机械通气时依从性和满意度的影响。方法:采用前瞻性研究方法将180例患者作为研究对象,其中A区90例作为对照组,B区90例作为观察组。2组均严格落实以"改模式、重临床、建机制"为核心的优质护理的各项措施,在"重临床"方面,观察组有创机械通气患者沟通时使用"一指明",对照组采用手语和语言相结合的传统方法。结果:观察组和对照组的人机协调满意率分别为80%、64%(P=0.029),保护性约束比例分别为73%、91%(P=0.002),意外拔管率分别为2%、10%(P=0.020)。满意度方面,观察组明显优于对照组(P=0.000),总体满意度观察组为60%,对照组为28%(P=0.000)。结论:沟通时使用"一指明"可以提高机械通气患者的依从性和满意度。
Objective To investigate the effect of picture instructions on the compliance and satisfaction of conscious patients undergoing invasive mechanical ventilation in high-quality care. Methods Totally 180 patients were included in this prospective study, of whom 90 patients in area A were used as control group, and the other 90 patients were in observation group. In both the groups, some measures were implemented as the core of the high-quality care including reformed service mode, emphasized clinical operation and complete mechanism. Picture instructions were available for the observation group, and the sign language and language were for the control group. Results The satisfaction rates for the man-machine cooperation were 80% and 64% respectively in the two groups with P value of 0.029, the protective confinement ratios were 73% and 91% respectively with P value of 0.002, and the rates for the unexpected drawn tubes were 2% and 10% respectively with P value of 0.020. The total satisfaction rate of the observation group was 60%, significantly higher than the 28% of the control group, with P value of 0.000. Conolusion The picture instructions can promote the compliance and satisfaction of the patients undergoing mechanical ventilation.
