地诺前列酮栓对足月引产产妇临床效果及安全性研究

Clinical efficacy and safety of dinoprostone suppository for induction of labor at term maternal

目的观察地诺前列酮栓用于足月妊娠的临床疗效及安全性。方法选取120例足月引产产妇并随机分为2组,每组60例。对照组产妇采用催产素引产,观察组产妇采用地诺前列酮栓引产。严密观察两组产妇引产过程中胎心、宫缩及产程进展情况,详细记录两组产妇用药前后宫颈Bishop评分、分娩方式、产妇不良反应、新生儿并发症发生率。根据两组产妇用药后Bishop评分情况进行疗效评定。结果观察组产妇总有效率为95.00%,对照组为75.00%,观察组疗效明显优于对照组(P<0.05)。观察组用药前宫颈Bishop评分为(4.1±0.5)分,用药后为(8.2±1.6)分;对照组用药前为(4.3±0.8)分,用药后为(4.8±0.8)分。观察组用药后评分明显高于用药前及对照组(P<0.05)。观察组阴道分娩率为93.33%,对照组为75.00%,观察组阴道分娩率明显高于对照组(P<0.05)。观察组和对照组新生儿并发症发生率分别为3.33%和5.00%,不良反应发生率分别为5.00%和8.33%,两组产妇新生儿并发症发生率及不良反应发生率比较差异均无统计学意义(P>0.05)。结论地诺前列酮用于足月引产可有效促进宫颈成熟,提高阴道分娩率,而且对胎儿无明显不良影响,药物不良反应发生率低。

Objective To observe the clinical efficacy and safety of dinoprostone suppository for full-term pregnancy. Methods 120 cases of full-term induction of maternal were randomly divided into two groups,60 patients in control group were given oxytocin, 60 patients in observation group were given dinoprostone. During induction of maternal fetal, the heart rate, uterine contractions and labor progress were observed. Bishop maternal score, mode of de- livery, maternal adverse reactions and the incidence of neonatal complications were recorded before and after treatment. Results The total effective rate of observation group was 95.00% , while it was 75.00% in control group （ P 〈 0. 05 ）. Before and after treatment, the cervical Bishop score of the observation group was （4. 1 ± 0. 5 ） and （ 8.2 ± 1.6） respectively,and the scores in control group were （4. 3 v 0. 8）and （4. 8 ± 0. 8）. The score of the observation group after treatment was significantly higher than those before treatment and the control group. The vaginal delivery rate of observation group was 93.33% , while it was 75.00% in control group（ P 〈 0. 05 ）. The neonatal morbidity rates of observation group and control group were 3.33 % and 5.00% respectively（ P 〉 0. 05 ）, the adverse reaction rates of the two groups were 5.00% and 8.33 % respectively （ P 〉 0. 05 ）. Conclusion Dinoprostone can effectively improve the rate of vaginal delivery, with no significant adverse effects on the fetus.
