透明质酸钠治疗颞下颌关节结构紊乱临床研究

Clinical research of sodium hyaluronate in the treatment of temporomandibular joint disorders

目的比较透明质酸钠与曲安奈德注射液治疗颞下颌关节结构紊乱的临床疗效。方法选取我院口腔科2009年3月至2011年9月收治的176例颞下颌关节结构紊乱患者的临床资料,其中,对照组88例患者采用曲安奈德注射液进行关节腔内治疗,观察组88例患者采用透明质酸钠进行治疗,比较、分析两组患者的临床疗效。结果治疗12个月后,观察组患者的优良率达70.45%,明显高于对照组的37.50%,两组比较差异有统计学意义(P<0.05);治疗后6、12个月,观察组患者骨关节病、内紊乱以及滑膜炎等症状改善率明显优于对照组,差异均有统计学意义(P<0.05)。结论透明质酸钠治疗颞下颌关节结构紊乱疗效显著,优于曲安奈德治疗效果,可明显提高患者治疗优良率,值得临床推广和应用。

Objective To compare the clinical efficacy of sodium hyaluronate and triamcinolone acetonide in- jection in the treatment of temporomandibular joint disorder. Methods 176 patients with temporomandibular joint dis- order were divided into two groups, control group （ n = 88 ） received intra-articular injection of triamcinolone acetonide, and observation group （ n = 88 ） received sodium hyaluronate treatment. The clinical effect of the two groups was com- pared. Results After 12 months of treatment,the excellent rate of observation group was significantly higher than that of control group （70.45 % vs. 37.50% ） , there was significant difference between the two groups （P 〈 0.05 ）. After 6 months and 12 months treatment,bone and joint diseases,internal disorders and synovitis symptom improvement of ob- servation group were significantly superior to those of control group,there were significant differences between the two groups （ P 〈 0.05 ）. Conclusion The clinical efficacy of sodium hyaluronate in the treatment of temporomandibular joint disorder is superior to triamcinolone acetonide treatment, and it is worth of clinical application.
