摘要 目的研究PET-CT在颞叶癫痫定位诊断中的应用和量化的可行性。方法选取27例确诊为颞叶癫痫患者与22例正常人,行头部18F-FDG PET-CT检查后,对比双侧颞叶葡萄糖摄取值的差值,观察颞叶癫痫患者病灶区代谢变化和正常人以及非病灶区域的差别。应用感兴趣区法(ROI法)寻找癫痫病灶,对比正常体检人群,统计双侧标准化摄取值(SUV)的放射性分布不对称指数(AI值)的变异范围。结果颞叶癫痫患者病灶侧的AI中位数为0.1607,癫痫患者病灶区域和非病灶区域,病灶区域与健康人群内侧和外侧差值均有显著差异,非病灶区域与健康人群差值无显著差异。结论颞叶双侧AI值0.1可以作为提示诊断癫痫灶的临界值。 Objective To assess the feasibility of PET-CT used in the localization of the epileptogenic focus. Meth- ods 27 patients who definitely diagnosed with temporal epilepsy and 22 healthy persons were enrolled. ISF-FDG PET-CT scan were performed in each patient. The differences of bilateral temporal glucose taken rate were explored between patients and healthy persons. Compare the changes of metabolism in temporal epilepsy patients and those in healthy person. Using ROI method to look for epileptic loci, and compare with normal person to calculate bilateral SUV difference variation range and explore the quantification index of lesion. Quantities index may be found to use in evaluate the localization of temporal lobe epilepsy. Result The median AI of epiieptogenie focus is 0. 1607. The variance between the epilepsy patients' tempo- ral lesions and normal side, the epilepsy patients' temporal lesions and bilateral lobe of healthy population lobe of healthy population were of significant difference. Conclusion AI value 0.1 can be used as a prompt diagnosis of epilepsy lesion.
