风湿性心脏病患者心功能状态及快速房颤对N-端脑钠肽前体浓度的影响 被引量：10

摘要 目的探讨风湿性心脏病(风心病)患者不同心功能状态下血清N-端脑钠肽前体(NT-proBNP)浓度的变化及快速房颤对它的影响。方法将58例风心病心力衰竭患者按美国纽约心脏病协会(NYHA)心功能分级标准随机分为Ⅱ级(n=19)、Ⅲ级(n=15)、Ⅳ级(n=24)组,各组再分为快速房颤亚组和非快速房颤亚组,18例心功能正常的风心病患者设为对照组,采用免疫金标法检测各组患者血清NT-proBNP的浓度并进行统计分析。结果与对照组相比,随着心功能分级增加,各组患者血清NT-proBNP浓度均明显增高(P<0.01),两两间比较差异均有统计学意义(P<0.01)。与非快速房颤亚组比较,各组内快速房颤亚组患者血清NT-proBNP浓度均明显增高(P<0.01),心功能Ⅳ级时,NT-proBNP浓度增高最明显。结论风心病心衰患者血清NT-proBNP浓度与心功能分级相一致;快速房颤对风心病心衰患者血清NT-proBNP浓度有明显影响,心功能Ⅳ级时,影响最明显。
