颈动脉内膜剥脱术感染控制的管理

Nursing management of prevention of infection during the carotid endarterectomy

目的探讨颈动脉内膜剥脱手术感染控制的管理要点,减少手术感染率。方法 2008年8月-2010年12月对17例颈动脉狭窄经颈动脉内膜剥脱手术,采用护理管理方法,对颈动脉内膜剥脱手术做好充分的术前准备及术中配合,分析其管理效果。结果 17例颈动脉内膜剥脱术的患者,术中应用有效的护理管理,术后均达到理想的护理效果,提高术中发生突发状况的应对能力,降低了并发症的发生,使患者在围手术期达到满意的护理效果。结论预防颈动脉内膜剥脱术的感染需要充分的术前准备和熟练的术中配合,术中全程监控手术器械及仪器无菌状态,对术中出现的出血等突发状况时,能迅速作出有效的护理对策,缩短了手术时间,减少了感染因素。

OBJECTIVE To investigate the key points in nursing management of the infection prevention during carotid endartereetomy, so as to reduce the rate of surgical infection. METHODS The nursing managements were taken for 17 patients with carotid artery stenosis undergoing arterial intima stripping operation from Aug. 2008 to Dec. 2010. The effect of management was analyzed on the basis of sufficient preoperative preparation and intraop-erative coordination. RESULTS A total of 17 patients achieved excellent outcome of nursing care by effectively management intraoperatively. The capadty of coping with the emergency during the operation was improved. The incidence of complications in those cases was reduced, and patients achieved satisfactory nursing during the periop-erative period. CONCLUSION To prevent infection of carotid endarterectomy, sufficient pre-operative preparation and skillful intra-operative cooperation are required. The sterility of surgical devices and instruments should be monitored during the whole operation. Effective nusing measures should be taken quickly for the emergency such as intraoperative bleeding. Operation time should be shortened to decrease the factors of infection.
