术中腹腔内温热灌注化疗综合治疗中晚期大肠癌的临床价值

COMBINED THERAPY OF SURGERY AND INTRAPERITONEAL HYPERTHERMIC PERFUSION CHEMOTHERAPY FOR ADVANC-ED COLORECTAI CANCER

目的:探讨术中腹腔内温热灌注(IPHP)化疗在中晚期大肠癌中的临床价值。方法:120例中晚期大肠癌病人随机分成A、B两组。A组68例行手术结合腹腔内温热灌注化疗;B组52例行单纯手术治疗。观察患者术中、术后临床表现,实验室检查和组织病理学变化,并进行随访分析。结果:两组术中、术后临床表现无显著差异,A组组织病理学改变明显,随访A组疗效优于B组。结论:术中IPHP化疗安全可行,对治疗大肠癌的腹膜转移效果显著,是控制中晚期大肠癌术后复发的一种有效方法。

ve: To determine the clinical value of intraperitoneal hyperthermic perfusion (IPHP) chemotherapy in preventing postoperative recurrence of advanced colorectal can-cer. Methods: One hundred and twenty patients with advanced colorectal cancer were randomly divided into 2 groups: In group 1, 68 cases were treated with IPHP chemotherapy; in the control group, 52 cases were only by operation. Clinic maniestation, histopathology and prognosis of group 1 and group 2 were compared. Results: The clinical manifestation has no significant difference be-tween the two groups in operation and post operation and post operation periods, but hispathology has an encouraging change in group 1. Follow-up survey showed that the effective of the therapygroup was better than that of the control group. Conclutions: Surgical operation combined with IPHP chemoterapy is safe, applicable and effective in preventing post operative recurrence in ad-vanced colorectal cancer, especially in the treatment of peritoneal metastasis.
