彩色多普勒检测胎儿大脑中动脉、肾动脉及腹主动脉血流阻力预测晚孕胎儿窘迫 被引量:3

摘要 目的探讨彩色多普勒超声对晚孕胎儿窘迫的诊断价值。方法选取2011年2月—2012年1月在该院做产前检查的住院或门诊孕妇154例,彩色多普勒超声检测108例正常晚期妊娠组和46例晚孕胎儿窘迫组胎儿大脑中动脉(MCA)、腹主动脉(AO)及肾动脉(RA)的血管收缩期峰值速度(S)与舒张末期流速(D)比值(S/D)、阻力指数(RI)。结果胎儿窘迫组MCA阻力指标均低于正常妊娠组(P<0.05),AO、RA阻力指标明显高于正常妊娠组(P<0.05),胎儿窘迫组AO、RA分别与MCA的S/D、RI关系密切,分别均呈直线负相关(P<0.05)。结论联合检测MCA及AO、RA的血流多普勒频谱参数,对预测晚孕胎儿窘迫有重要临床意义。 Objective To study the value of color Doppler ultrasonography in the diagnosis of fetal distress in women of late pregnancy. Methods 154 inpatient or outpatient pregnant women underwent prenatal examination in our hospital from February, 2011 to January, 2012 were selected. Ratio of peak systolic velocity and end diastolic velocity(S/D) and resistance index(RI) were detected by color Doppler blood flow imaging in middle cerebral artery(MCA), aortaventralis(AO) and renal artery(RA) of 108 normal fetuses and 46 distress fetuses in women of late pregnancy. Results The S/D and RI of MCA were lower while those of AO and RA were higher in normal fetuses than in those with distress(P0.05). The S/D and RI of MCA were closely related to those of AO and RA in fetuses with distress respectively(P0.05), with negatively related. Conclusion United detection of MCA and AO, RA blood flow Doppler spectrum parameters is of great importance in predicting fetal distress in late pregnancy.
