恶性萎缩性丘疹病一例

Malignant atrophic papulosis:a case report

报道1例恶性萎缩性丘疹病。患者男,58岁。因全身皮损5年余,急腹症剖腹探查术后11 d入院。躯干、四肢散在分布圆形、椭圆形淡红色丘疹,黄豆大小,周边呈环状隆起,境界清楚,中央破溃、萎缩。皮损组织病理示真皮深部小血管周围淋巴细胞浸润,部分小血管管腔闭塞。临床表现及组织病理改变符合恶性萎缩性丘疹病。该病临床罕见,累及系统,死亡率高。

To report one case of malignant atrophic papulosis. A 58-years-old man presented a 5-year history of asymptomatic erythraeous papules over the trunk and limbs. Physical examination showed the lesions were pink dome-shaped papules, some of which presented with a peripheral telangiectatic rim and a small porcelain white central atrophic scar. Skin biopsies revealed the occlusion of blood vessels in the dermis and perivascular lymphohistiocytic inifltration. The diagnosis of malignant atrophic papulosis（MAP） was made based on the characteristic clinical presentation and histopathological ifndings. The disease is dififcult to deal with when systemic involvement developed and with a high mortality.
