联合应用包皮与阴囊独立皮瓣一期修复尿道下裂的探讨

One stage urethral reconstruction of hypospadias by two separate pedicled skin flaps from prepuce and scrotum

目的 :探讨阴囊及会阴型尿道下裂患者一期修复重建尿道的新术式。方法 :联合应用包皮与阴囊两独立带蒂皮瓣施行尿道重建术一期修复阴囊及会阴型尿道下裂患者 13例。结果 :11例获得成功 ,2例术后出现尿瘘 ,其中 1例经尿道扩张治疗 ,3周后尿瘘愈合 ,另 1例行二期尿道成形术后尿瘘痊愈。结论 :联合应用包皮与阴囊两独立皮瓣重建尿道的优势明显 ,更多地集中了目前治疗阴囊及会阴型尿道下裂众多术式的优点 ,获得满意疗效。

Purpose:One stage urethral reconstruction of scrotal and perineal hypospadias was performed in order to renew normal appearance and micturiton of urethra.Methods:Combination of taking preputial and scrotal pedicled skin flaps respectively was carried out to reconstruct the urethra of hypospadias.Results:13 cases of scrotal and perineal hypospadias were operated,11 cases of them have got successful results from one stage operation. The other 2 cases had urinary fistula after reconstructive procedures. One was cured by soundings for 3 weeks. Another fistula was cured by second stage urethroplasty. Conclusions:The operating style of urethral reconstruction by use of separate preputial and scrotal pedicaled flaps have not been reported at home. The superiorities of the discussing procedure are properly able to concentrate the advantages of different operating styles of urethral reconstruction of scrotal and perineal hypospadias, which have been performing recently. In general, the operating results are exciting.
