三切口联合入路切开复位内固定治疗复杂胫骨平台骨折 被引量:6

摘要 目的探讨三切口联合入路切开复位内固定治疗复杂胫骨平台骨折的手术方法及疗效。方法回顾性分析2008年3月至2011年4月收治的18例单侧累及后侧平台的胫骨平台骨折患者,男12例,女6例;年龄24~55岁,平均34.7岁;骨折按照Schatzker分型:V型8例,VI型10例,所有患者均合并后内、后外侧平台塌陷。均采用三切口联合入路切开复位内固定,术后定期复查x线片,观察骨折愈合情况。结果所有患者均获随访,时间6~33个月(平均14个月),根据X线检查判断骨折均获愈合,愈合时间15~20周(平均17.6周)。膝关节按照美国纽约特种外科医院(HSS)评分评定疗效:优13例,良4例,可1例。所有患者均无神经、血管损伤、筋膜间隔综合征、骨髓炎、内固定松动、断裂、伤口感染等并发症发生。结论三切口联合入路切开复位内固定治疗复杂胫骨平台骨折可以充分暴露骨折,满意复位关节面,固定可靠,可早期功能锻炼,患肢术后功能恢复良好。
