摘要
目的:探讨人工骨粉在与邻牙根影像重叠多生牙摘除中应用的临床效果。方法:选择2009~2012年经治的与邻牙根影像重叠的多生牙7例8颗牙,分成两组。两组均采用弧形切口翻瓣凿骨后完整拔除埋伏多生牙。骨粉组4例4颗牙拔牙后,牙槽窝即刻植入Bio—Oss人工骨粉和Bio—Gide生物膜;对照组3例4颗牙不植入Bio—Oss骨粉和生物膜,仅将新鲜血液充满骨创即关闭手术切口。术后1、2周及1、3、6、12月复查,观察临床效果、x线片检查骨愈合情况、记录邻牙松动度及牙髓活力测试。结果:骨粉组邻牙均未出现松动、无咀嚼疼痛及继发牙髓炎的症状,x线片显示根尖牙周膜间隙正常,无暗影。对照组4例均有不同程度的手术创区邻牙伸长感,咬合痛显著,x线显示根尖牙周膜间隙不同程度增宽,1例邻牙松动,1例继发牙髓炎症状。结论:人工骨粉在邻牙根影像重叠多生牙拔除中应用的临床效果明显。
Objective:to explore the clinical effect of artificial bone in application of extracting supernumerary tooth which is ovedaping with adjacent root image. Method:The supernumerary teeth overlaping with adjacent root image were chosen,divided into two groups.The teeth in two group were both completely extracted afer being arc-shaped flapped and re- moved bone. The experimental group were 4 cases of 4 impacted teeth,Bio-Oss artificial bone and Bio-Gide periosteum were instantly implanted into the sockets in the group after the teeth extracted. The control group were 3 cases of 4 impacted teeth,the sockets in the group were only treated with new blood after the teeth extracted. The clinical effects were observed, bone healings were examined with X-ray,adjacent teeth mobility and dental pulp vitality test were recorded on 1 and 2 weeks,1,3,6 and 12 months after the operation. Result:The adjacent teeth in group of artificial bone have not become loose, have no pain when chew, have no symptoms of pulpitis. X-rays show that their root tip and parodontium are normal and have no shadows. The 4 cases of adjacent teeth in the control group all have elongation feeling, pain obviously when bite,X- rays show that their root tip and parodontium become broadening at different levels.;1 case of adjacent teeth becomes loose; 1 case of adjacent teeth appears secondary pulpitis. Conclusion:The clinical effect of artificial bone in application of extract- ing supernumerary tooth overlaping with adjacent root image is significant.
