彩色多普勒超声诊断早期小儿肠套叠32例分析

Color Doppler Ultrasound in Early Diagnosis of Intussusception in Children:Analysis of 32 Cases

目的探讨彩色多普勒对小儿肠套叠的诊断及辅助临床选择复位方法的价值。方法对32例小儿肠套叠的病史,临床表现,临床复位结果等资料进行回顾性分析,总结超声影像学特征。结果超声诊断32例小儿肠套叠,28例超声引导下生理盐水灌肠复位成功(87%),4例手术复位成功(12%),阻力指数(RI)显著升高,无血流显示等情况时应尽快手术复位。结论彩色多普勒超声检查具有诊断率高,无痛、无创、能测血流并可多切面进行连续地动态观察等特征,是诊断肠套叠的首选影像诊断方法。

Objective To investigate the value of color Doppler flow imaging（CDFI） for diagnosing children＇s intussusceptions and its significance for choosing the way of reduction. Methods To review and analyze medical history, clinical manifestation, results of clinical retroposition of all 32cases, and summarize the characteristics of CDFI. Results A total of 32 cases with intussusception were diagnosed with ultrasonography. Water enema for reduction of intussception under ultrasonography guidance were performed in 28 cases（87%）. 4 cases（12%） were confirmed by operative reduction. Operative reduction should be performed quickly when significantly high RI, especially no blood flow imaging are demonstrated in color Doppler ultrasonography. Conclusion The CDFI has the advantage of high accuracy, no pain, noninvasive, measurement of blood flowing,continuous and dynamic observation from multiplannar cross sections.
