术前短期放疗联合腹腔镜根治直肠癌的手术效果观察

Surgery Effect of Short-term Preoperative Radiotherapy Combined with Laparoscopic Radical Colorectal Cancer

目的探讨术前短期放疗联合腹腔镜根治直肠癌手术的临床效果,为治疗直肠癌提供临床依据。方法对2008年6月至2011年6月来我院治疗Dukes B和C期直肠癌的120例患者进行回顾性分析,随机分成A、B、C三组,每组40例,A组采用术前短期放疗联合腹腔镜方式,B组采用未短期放疗联合腹腔镜方式,C组采用未短期放疗联合开腹手术方式,观察三组的治疗效果。结果术前短期放疗联合腹腔镜方式根治直肠癌手术切除率和保肛率明显高于未短期放疗联合腹腔镜方式以及未短期放疗联合开腹手术方式,存在统计学意义(P<0.05);术前短期放疗联合腹腔镜方式根治直肠癌手术远处转移率、局部复发率和并发症发生率明显低于未短期放疗联合腹腔镜方式以及未短期放疗联合开腹手术方式,存在统计学意义(P<0.05)。结论术前短期放疗联合腹腔镜根治直肠癌手术具有很好的保肛率和根治率,在临床上可大力推广。

Objective To explore the clinical effect of laparoscopic radical operation for rectal cancer after preoperative short-time radiotherapy, and to provide clinical evidence for the treatment of rectal cancer. Methods 120 patients of our hospital Dukes B and C rectal cancer from June 2008 to June 2011 were analyzed retrospectively. A, B and C were randomly divided into three groups, with 40 cases in each group, and observe the therapeutic effect of the three groups. Results There was a higher rate of radical resection and sphincter preservation in the after preoperative short-time radiotherapy combined with laparoscopic group than in the other groups,and there are signiifcant（P〈0.05）;At rate of distant metastasis, rate of local recurrence and rate of complication, there were also lower rates of local recurrence and distantmetastasis in the laparoscopic radiotherapy group than in the other groups, and there are signiifcant（P〈0.05）.Conclusions Preoperative short-term radiotherapy combined with laparoscopic radical cancer surgery has higher sphincter preservation rate and cure rate, and it can be popularized.
