摘要
目的通过记录4种不同人工关节置换手术后C反应蛋白(CRP)的变化趋势及相关水平来研究这4种不同手术对机体造成的创伤反应。方法研究96例不同关节置换(全膝关节置换、单髁膝关节置换、表面髋关节置换、全髋关节置换)患者,分别记录其术前、术后2d、术后7d的CRP水平。结果 CRP的峰值在术后第2天出现,表面髋关节置换组峰值低于全髋关节置换组,单髁膝关节置换组峰值低于全膝关节置换组,全髋关节置换组峰值低于全膝关节置换组。结论术后CRP的反应程度是由手术过程中骨髓组织的破坏程度决定的,不是由手术部位或软组织的损伤程度决定的。
Objective To research the trauma response of 4 types of arthroplasty by recording the CRP levels and asso- ciated trends of 4 types of arthreplasty. Methods Investigated 96 patients who had total hip arthroplasty (THA), total knee arthreplasty (TKA),hip resurfacing arthroplasty (metal on metal,MMSA) and unicompartmental knee arthroplasty (UKA) re- spectively. CRP levels were estimated preoperatively and postoperatively at the second and the seventh days. Results Postop- eratively,the peak CRP level was the highest at the second day after surgery in each groups. The peak CRP level after hip resur- facing arthropalasty was lower than that after total hip arthroplasty. The peak CRP level after unicompartmental knee arthroplasty was lower than after total knee arthroplasty,the peak CRP level after total hip arthroplasty was lower than after total knee arthro- plasty. Conclusion Postoperative reaction degree of CRP level is determined by the bone marrow tissue damage in the pro- cess of parthroplasty,not determined by the arthroplasty site or the damage of soft tissue.
