芪苈强心胶囊在高龄慢性心衰患者中的临床疗效

The clinical effect of Qiliqiangxin capsules in treatment of elderly patients with chronic heart failure

目的探讨芪苈强心胶囊在高龄患者慢性心衰中的临床疗效。方法 84例高龄慢性心衰患者随机分为治疗组(42例)和对照组(42例),两组均给予优化抗心衰治疗,治疗组加用芪苈强心胶囊口服,每次4粒,3次/d,两组均12周为一个疗程。观测治疗前后心功能、6 min步行距离及相关指标变化,记录不良反应。结果两组治疗后NT-proBNP、明尼苏达心衰生活质量评分及NHYA分级较治疗前改善,治疗组改善明显优于对照组(P<0.05-0.001)。治疗前后两组的左室射血分数(LVEF)、左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)、心输出量(CO)等指标比较,差异均具有统计学意义(P<0.05),治疗组治疗后LVEF与对照组比较差异有统计学意义(P<0.01)。治疗组6 min步行距离与对照组比较有显著差异[(358±42.3)m vs(201±41.0)m,P<0.05]。结论高龄慢性心衰患者在标准优化抗心衰治疗的同时,加用芪苈强心胶囊有助于改善患者的心功能,提高生活质量。

Objective To explore the clinical effect of Qiliqiangxin capsules in treatment of elderly patients with chronic heart failure （CHF）. Methods Eighty-four elderly patients with chronic heart failure were randomly divided into treatment group （42 cases）and control group （42 cases ）.Both groups were received optimized therapy according to guideline for chronic heart failre to improve cardiac function. Meanwhile lrestment group received additionally Qiliqiangxin capsule （4 granules each time,three times a day）.All patients＇ therapentie. course was 12 weeks,then observed the cardiac function, 6 min walking distance and related indexes between the pretreatment,post treatment,and record side effects were observed. Results After treatment, two groups N-terminal pro-B-type natriuretic peptide（NT- proBNP） level,the Minnesota heart failure quality of life scores and NYHA functional classification was improved than before,treatment group improved significantly better than the control group （/~〈0.05 or 0.001 ）. Compared with the left ventrieular ejection fraetion（LVEF）,left ventricular end systolic diameter（LVESD）, left ventricular end diastolic di- ameter （LVEDD）,the cardiac output （CO）between the pretreatment and post treatment group,the differences of these indexes were statistical significant（P〈0.05）,and LVEF improved significantly better than control group（P〈0.01）.After 12 weeks therapy, the treatment group 6 rain walking distance compared with control group had significant differences [ （ 358±42.3 ） m vs （201±41.0 ） m, P〈0.05]. Conclusion Qiliqiangxin capsule combined with conventional medicine could effectively improve cardiac function in elderly patients with chronic heart failure ,enhance the quality of life.
