微创三角肌外侧入路结合解剖锁定加压钢板内固定治疗肱骨近端Neer Ⅱ、Ⅲ型骨折的效果分析

Efficacy analysis of the minimally-invasive lateral approach deltoid combined with anatomical locking compression plate fixation in treatment of proximal humerus Neer Ⅱ,Ⅲ type fractures

目的探讨微创三角肌外侧入路结合解剖锁定加压钢板内固定治疗肱骨近端NeerⅡ、Ⅲ型骨折的疗效。方法将100例肱骨近端NeerⅡ、Ⅲ型骨折患者按照抽签方法随机均分为对照组与观察组,对照组给予肱骨近端锁定钢板内固定,观察组在此基础上给予微创三角肌外侧入路进行治疗,比较两组的Neer功能评分、手术时间、术中出血量、住院天数及骨折愈合时间等。结果根据Neer功能评价标准,对照组临床治疗优良率为62.00%,观察组为92.00%,两组差异有统计学意义(P<0.01);观察组手术时间、术中出血量、住院天数及骨折愈合时间均少于对照组,差异有统计学意义(P<0.05,P<0.01)。结论微创三角肌外侧入路结合解剖锁定加压钢板内固定治疗肱骨近端NeerⅡ、Ⅲ型骨折疗效显著,安全性较高,值得临床推广应用。

Objective To investigate the efficacy of the minimally-invasive lateral approach deltoid combined with anatomical locking compression plate fixation in treatment of proximal humerus Neer Ⅱ、Ⅲ type fractures.Methods 100 cases of patients with proximal humerus Neer Ⅱ、Ⅲ type fractures were randomly divided into control group and observation group according to balloting.The control group were given locking proximal humerus plate fixation,and the observation group were given minimally-invasive lateral approach deltoid combined with anatomical locking compres- sion plate fixation on the basis of the method of the control group.The Neer function scores,operative time,blood loss, days of hospital ＇staying and fracture healing time was compared between two groups.Results According to Neer func- tional evaluation criteria,the clinically excellent rate of the control group was 62.00% and the rate of the observation group was 92.00%,the difference was statistical between the two groups （P〈0.01）.The operation time,blood loss,hospital stay and fracture healing time of the observation group was less than that of the control group respectively,with statisti- cal difference （P〈0.05,P〈0.01）.Conclusion Minimally-invasive lateral approach deltoid combined with anatomical locking compression plate fixation in treatment of proximal humerus Neer Ⅱ、Ⅲ fracture has significant efficacy,high security,and it deserves to be promoted and applied in clinical treatment.
