福建省美沙酮维持治疗门诊现状分析

ANALYSIS ON THE CURRENT STATUS OF METHADONE MAINTENANCE TREATMENT CLINIC IN FUJIAN PROVINCE

目的:了解福建省美沙酮维持治疗工作现状,分析存在的问题,为制定发展规划提供科学依据。方法:通过问卷调查、查阅艾滋病综合防治信息系统等形式,了解各门诊工作开展情况。结果:截至2012年6月底,全省共建立18个美沙酮维持治疗门诊,累计入组病人7980例,在治病人2438例;病人日均服药剂量50.0 mg,尿检阳性率24.7%;有42.5%医务人员已离岗,有13个门诊配备的工作人员数达不到国家要求;有12个门诊处于亏损状态。结论:福建省美沙酮维持治疗病人脱失率高、门诊运营资金不足、医务人员流动性过大、家庭/社区参与度不高。下一步工作重点应降低脱失率,加大资金投入,完善多部门合作机制,加强综合干预和家庭/社区帮教。

Objective：To study the current situation of methadone maintenance treatment （MMT） and analyze the problems exiting in MMT, so as to provide scientific basis for making development programs. Methods：The process of MMT clinics was investigated by questionnaire surveys and reviewing the information system of HIV/AIDS comprehensive prevention and control..R.esul._ts_： By the end of June 2012, there had been 18 MMT clinics in Fujian province, 7980 patients had been enrolled in 18 clinics in total, and 2438 patients continued doing treatment. The average dosage was 50.0 mg per day, and urine test positive rate was 24.7%. There were 42.5% medical staff left, and 13 clinics of which the numbers of staff did not meet the national requirement. In addition, 12 clinics lost money. Conclusion.： The rate of patient dropping out was high, and the operation funding of clinic was not enough. Also, the problem of staff instability was serious, family and community participation was not active. It is necessary to reduce the rate of patient dropping out, increase the funding, perfect the multi - sectorial cooperation mechanism, and strengthen comprehensive intervention and family/community education in next step.
