微创经皮肾镜碎石术处理分支肾盂肾结石的手术技巧探讨

Investigation of operation skill treating renal calculus of branch renal pelvis with invasive percutaneous nephroscopic lithotripsy

目的探讨微创经皮肾镜碎石术处理分支肾盂肾结石的手术方法与技巧。方法回顾性分析2011年6月-2012年12月河北大学附属医院收治的30例分支肾盂肾结石行经皮肾镜碎石术处理患者的临床资料，术前经静脉肾盂造影及CT平扫加成像确诊，结石位于分支肾盂的下支27例，上支3例。结果30例患者均一期成功建立通道行碎石术，术后无发热、出血等并发症。一期结石清除率约为93％，2例因结石残留行二次碎石术。结论微创经皮肾镜碎石术是处理分支肾盂肾结石的有效手段，清石率高，安全性好，值得临床广泛推广。

Objective To investigate the operatinal method and skill of treating renal calculus of branch renal pelvis with minimally invasive pereutaneous nephroscopie lithotripsy. Methods Clinical data of 30 patients with renal calcu- lus of branch renal pelvis treated by percutaneous nephroscopic lithotripsy in affiliated hospital of Hebei university from June 2011 to December 2012 were retrospectively analyzed,all patients were diagnosed by intravenous pyelogra- phy,CT scanning and imaging before operation,among them, calculus was located in the upper branch of branch renal pelvis in 27 cases while it was located in the lower upper branch of branch renal pelvis in 3 cases. Results 30 pa- tients were successful established channel for lithotripsy,and there was no fever, bleeding and other complications after operation.The stone clearance rate was about 93% in the first period,2 cases were conducted with the second lithotfip- sy because of residual stones. Conclusion Minimally invasive percutaneous nephroscopic lithotripsy was an effective means treating renal calculus of branch renal pelvis,and its stone clearance rate is high and it has good.h is worth widely promoting.
