螺内酯治疗老年高血压并发舒张性心力衰竭疗效分析

Analysis of Curative Effect of Spironolactone in the Treatment of Senile Hypertension and Diastolic Heart Failure

目的分析应用螺内酯治疗老年高血压并发舒张性心力衰竭的临床疗效。方法基础治疗前提下,配合应用螺内酯治疗观察组的84例高血压并发舒张性心力衰竭老年患者;并与基础治疗的76例对照组患者治疗后心脏指标等进行对比。结果治疗后,测量患者6min步行结果,两组患者步行距离均长于治疗前,患者运动耐受性提高,观察组治疗后步行结果为(542±56)m,显著大于对照组(434±45)m,结果具有统计学意义(P<0.05);观察组EPFV、APFV、EPFV/APFV、DT值明显改善,患者血浆内脑尿钠肽、醛固酮、肾素及血管紧张素Ⅱ水平均下降,其中脑尿钠肽下降幅度最大,与对照组对比,结果具有统计学意义(P<0.05)。结论利尿剂螺内酯可有效提升伴有DHF的高血压老年患者运动耐受能力,具有改善心脏功能的作用。

Methods Foundation treatment under the premise, combined with application of spironolactone treatment group of 84 cases of hypertension complicated with diastolic heart failure in elderly patients; And after the foundation treatment of 76 cases of the control group patients treated cardiac index,etc. Results After treatment, the patients with 6 minutes walk as a result, the two groups patients walking distance is longer than before the treatment, exercise tolerance in patients with increased,observation group after treatment results of （542±56） meters on foot, significantly greater than the control group（434±45）m, the result was statistically significant （P〈0.05）; Observation group EPFV and APFV and EPFV/APFV and DT value is obviously improved, patients with brain urine sodium peptide in plasma, blood vessels, renin and aldosterone Wei Yang 11 level decreased, the brain peptide decreased urine sodium largest,compared with control group,the result is statistically significant （P〈0.05）. Conclusion Diuretic spironolactone can effectively improve the tolerance of hypertension in elderly patients with DHF movement ability, improve the function of the heart.
