锁定钢板治疗青少年胫骨远端三平面骨折的临床效果

Clinical effect of locking plate in the treatment of adolescent distal tibial triplane fracture

目的探讨锁定钢板治疗青少年胫骨远端三平面骨折的临床效果，以评价其临床价值。方法回顾性分析2009年1月～加12年1月本院骨科收治的100例青少年胫骨远端三平面骨折患者的临床资料，按随机数字表法将其分为观察组与对照组，每组50例，观察组患者给予锁定钢板内固定法治疗，对照组患者给予传统切开复位内固定法治疗，比较两组患者的平均愈合时间、并发症发生率及治愈率。结果观察组患者平均愈合时间为4-4个月，对照组患者平均愈合时间为6．5个月，差异有统计学意义（P〈0．05）；两组患者并发症发生率比较差异无统计学意义（P〉0．05）；观察组患者治愈率明显高于对照组，差异有统计学意义（P〈0．05）。术后随访1年，所有患者内固定均未出现松动或断裂，关节未出现卡压，骨折均愈合良好，未出现关节退变、短缩等情况。结论锁定钢板可有效治疗青少年胫骨远端i平面骨折，其创伤小、安全可靠、无并发症发生，值得临床推广应用。

Objective To investigate clinical effect of locking plate in the treatment of adolescent distal tibia1 triplane fracture in order to evaluate its clinical value.Methods The clinical datas of 100 cases with orthopedic teenagers tri- plane fracture of the distal tibia were analyzed retrospectively.They were randomly divided into observation group and control group,each group of 50 patients.Observation group was treated with locking plate fixation,control group was treated with traditional open reduction and internal fixation.The average healing time,incidence rate of complication,the cure rate in two groups were compared.Results The average healing time of observation group was 4.4 months,the aver- age healing time of control group was 6.5 months,with statistical difference （P〈O.OS）.Incidence rate of complication in two groups was compared,with no statistical difference （P〉O.OS）.The cure rate of control group was significantly higher than that of control group,with statistical differcnce（P〈0.05）.After 1 years follow-up,all patients had no internal fixation loosening or fracture,joint without compression,fracture healed well,no joint degeneration and shortening.Conclusion The locking plate is effective in the treatment of distal tibia in teenager triplane fracture.Its little trauma,safe and reli- able,no complication.h is worthy of clinical application.
