摘要 目的研究J波改变的病因、心电图特点及临床意义。方法回顾性分析2005年8月~2012年3月资料较完整、诊断明确的56例住院患者J波改变。分析病因及心电图特点。结果56例患者根据病因不同分为:低温性J波3例,2例患者心电监测发现多形性室性心动过速及心室颤动,抢救无效死亡;高血钙J波2例;神经性J波16例,3例患者出现多形性室性心动过速及心室颤动,死亡;早期复极综合征J波13例;缺血性J波22例,其中2例患者死亡。结论J波改变与恶性心律失常发生有一定的关系,早期识别高危J波改变患者,根据临床情况进行积极有效的治疗,包括植入埋藏式心脏转复除颤器,防止猝死发生,可有效地改善患者预后。 Objective To study the etiology,electrocardiogram characteristics and clinical significance of J wave changes. Methods Causes and characteristics of electrocardiogram of 56 cases of hospitalized patients with definite J wave changes from August 2005 to March 2012 were retrospectively analyzed.Results In the 56 patients, there were 3 cases with low temperature J wave,2 cases of polymorphous ventricular heartbeat tachycardia and ventrieular fibrillation,mon- itored by ECG,who died during the rescue; 2 cases of hypercalcemia J wave; 16 cases of neural J wave,3 patients died of polymorphie ventricular taehyeardia and ventricular fibrillation,13 cases of early repolarization syndrome J wave; 22 cases of ischemic J wave,of whom 2 patients died.Conclusion J wave changes and malignant arrhythmia have a certain relationship.Early identification of high-risk patients with J wave changes,active treatment according to the clinical sit- uation,including the implantable cardioverter defibrillator,can prevent the occurrence of sudden death,effectively im- prove the prognosis.
