早期股骨头坏死CT与MRI诊断价值的比较研究

Comparative study on diagnostic value of CT and MRI in early osteonecrosis of femoral head

目的探讨CT与MRI对早期股骨头坏死的诊断价值。方法选择本院2010年3月～2013年6月收治的股骨头坏死患者38例（62髋），患者均同时行CT及MRI诊断，并采用ARCO分期方法评价CT与MRI对股骨头坏死的早期诊断价值，比较两种方法典型征象的显示率。结果I～Ⅱ期股骨头坏死MRI的检出率明显高于CT（P〈0．05），两者对Ⅲ、Ⅳ期股骨头坏死的检出率比较，差异无统计学意义（侈0．05），两者对股骨头坏死的总检出率比较，差异有统计学意义（P〈0．05）；在早期（I～Ⅱ期）股骨头坏死中，MRI对线样征、骨髓水肿、骨小梁结构模糊的显示率明显高于CT（P〈0．05），两者对局部囊变的显示率比较，差异无统计学意义（P〉0．05）。结论MRI在早期股骨头坏死诊断中的价值要明显优于CT，尤其对线样征、骨髓水肿等典型征象的显示率较高，值得应用。

Objective To investigate the diagnostic value of CT and MRI in early osteonecrosis. Methods 38 patients （62 hips） with osteonecrosis of femoral head were selected in our hospital from March 2012 to June 2013,while all pa- tients were given CT and MRI examination,and the ARCO staging method was used to evaluate diagnostic value of CT and MRI in early osteonecrosis of femoral head,and typical signs detection rate of two methods were compared. Results The detection rate of osteonecrosis of femoral head in stage [ - II in MRI was significantly higher than that in CT （P〈O.05）, the detection rate of osteonecrosis in stage m, 1Vof both methods was compared and there was no statistical difference between the two methods （P〉0.05）,the total detection rate of osteonecrosis of femoral head of the two methods was com- pared and there was a statistical difference between the two methods （P〈0.05）;in early osteonecrosis of femoral head （stage [ - U ）,the display rate of the string sign,bone marrow edema,trabecular structure blur in MRI was significantly higher than that in CT （P〈0.05）,the display rate of the local cyst of the two groups was compared,and there was no statistical difference between the two methods （P〉0.05）. Conclusion The diagnostic value of MRI in the early diagnosis of osteonecrosis of femoral head is superior to CT,especially the display rate of string sign,bone marrow edema and oth- er typical signs is higher,it is worthy of application.
