多层螺旋CT对肺内孤立性结节的诊断及其鉴别诊断价值

Value of multi slice spiral CT in the diagnosis and differential diagnosis of solitary pulmonary nodules

目的探讨多层螺旋CT对肺内孤立性结节的诊断及其鉴别诊断价值。方法2011年5月～2013年5月，本院应用飞利浦64层螺旋CT诊断肺内孤立性结节180例，对其进行定期随访，将CT诊断结果与随访的病理结果进行比较分析。结果180例肺内孤立性结节的CT检出率为100％，其中良性结节120例，恶性结节60例，良性结节的CT诊断结果与病理结果符合的118例，符合率为98．3％。恶性结节的CT诊断结果与病理结果符合的为57例，符合率为95．0％；良恶性结节的结节大小与影像学特征比较，差异有统计学意义（P〈0．05）。结论多层螺旋CT对肺内孤立性结节良恶性的鉴别诊断具有极高的临床价值，值得在基层医院应用推广。

Objective To investigate the value of multi slice spiral CT in the diagnosis and differential diagnosis of solitary pulmonary nodules. Methods 180 cases of solitary pulmonary nodules were diagnosed by 64 layers spiral CT of Philips in our hospital from May 2011 to May 2013,all patients were regularly followed-up,the CT results and patho- logical results of follow-up was compared and analyzed. Results The detection rate of CT in 180 cases of solitary pul- monary nodules was 100%,among them,120 cases were benign nodules,60 cases were malignant nodules, 118 cases of benign nodules of CT diagnosis were consistent with pathological results and the coincidence rate was 98.3%,57 cases of malignant nodules of CT diagnosis were consistent with pathological results and the coincidence rate was 95.0%; there was a statistical difference in size and imaging features between benign and malignant nodules （P〈0.05） . Conclusion Multi slice spiral CT has high clinical value in differential diagnosis of benign and malignant solitary pul- monary nodules,and it is worth popularizing and applying in basic-level hospitals.
