儿童扁桃体肥大低温等离子消融术观察

Investigate on coblation plasma surgery of tonsillar hypertrophy in children

目的 探讨扁桃体低温等离子打孔消融术的疗效及可行性.方法 扁桃体肥大患儿58例,所有患者均在全麻下用低温等离子Reflex 55刀头,分别沿扁桃体上、中、下极部插入,每次每点消融15秒,退出时凝血,视扁桃体大小分别打4～6点.其中,27例同时切除腺样体.结果 扁桃体均较术前有不同程度缩小,缩小至Ⅰ度,临床症状消失47例（81％）;由Ⅲ度缩小至Ⅱ度,临床症状减轻8例（13.8％）;扁桃体无明显缩小3例（5.2％）,为伴扁桃体炎患者.结论 扁桃体低温等离子打孔消融术可作为治疗扁桃体肥大的方法之一,并有明显的优势;但伴扁桃体炎时选择要慎重.

Objective To Investigate curative effect and feasibility of coblation plasma surgery of tonsils. Methods 58 children patients with tonsillar hypertrophy were operated with coblation plasma surgery （Reflex 55）. Every tonsil was 4 -6 location, it kept 15s in every location. Results The tissue volume reduction of tonsils was dissimilar, 47 patients were cured （81% ）, and 8 patients were improved （ 13.8 ）, and 3 patients with chronic tonsillitis were ineffectual （5.2%）. Conclusions Coblation plasma surgery of tonsils is one mode of cured tonsillar hypertrophy. But patients with chronic tonsillitis are gingerly.
