耐药性肺结核的手术治疗(附36例报告)

Surgical treatment of multidrug resistance pulmonary tuberculosis

目的 探讨耐药肺结核的手术适应症及手术时机。方法 对 1990～ 1997年我院收治的 70 0 0肺结核和 36例耐药肺结核已手术治疗的病人的手术效果进行回顾分析。结果 36例肺结核病人 ,临床治愈率 94 % ,痰菌阴转率 84 % ,并发症发生率为 13%。结论 对部分耐 2～ 3种药的肺结核病人选择外科治疗仍是可行的 ,可提高耐药肺结核的治愈率 ,耐 4种以上的肺结核病人术后并发症发生率高 。

Objective To discuss the operation indication for pulmonary tuberculosis with drug resistance.Methods Reviewing 7 000 cases of pulmonary tuberculosis in which 36 cases with drug resistance that have been operated in our hospital from 1990～1997.Results The cure rate of 36 cases of pulmonary tuberculosis with drug resistance is 94%,sputum negative conversion rate is 84%,and the incidence rate of complication is 13%.Conclusion It is feasible for some patients who have 2～3 kinds of drug resistance to receive surgical treatment.It can improve the cure rate of drug resistance pulmonary tuberculosis but the complication rate of patients who have at lease 4 kinds of drug resistance is high,so that more prudent indication criteria should be considered.
