摘要
目的 探讨血管紧张素转换酶 (ACE)基因I/D多态性与阻塞性睡眠呼吸暂停综合征 (OSAS)及OSAS合并高血压的相关性。方法 采用PCR扩增方法 ,对 80例OSAS患者进行ACE基因多态性检测及多导睡眠图监测。选择 6 0例健康人做对照。结果 正常人群中ACE基因II、ID、DD型的分布频率分别为 2 5 %、 5 0 %、2 5 % ,OSAS患者为 5 2 %、 38%、 10 % ,两组相比其构成有显著性差异 (χ2 =12 31,P <0 0 5 )。正常人群中D和I等位基因的频率均为 5 0 % ,OSAS患者为 2 9%和 71% ,两组相比其分布有显著性差异 (χ2 =4 83,P <0 0 5 )。单纯OSAS患者等位基因D和I的频率分别为 36 %和 6 4% ,OSAS合并高血压患者为 2 2 %和 78% ,其分布有显著性差异(χ2 =3 796 ,P <0 0 5 )。OSAS合并高血压患者较单纯OSAS患者其平均呼吸暂停时间更长 (P <0 0 5 )、呼吸暂停低通气指数 (AHI)更高 (P <0 0 5 )、最低氧饱和度 (SaO2 )更低 (P <0 0 5 )、SaO2 <90 %的次数更多 (P <0 0 5 )。II基因型患者与ID及DD基因型患者相比 ,AHI更高 (P <0 0 5 )、最长呼吸暂停时间更长 (P <0 0 5 )、SaO2 <90 %的次数更多 (P <0 0 5 )。结论 OSAS的发病与ACE基因I/D多态性相关联。基因型II和等位基因I是OSAS发病的危险因素 。
Objective To investigate the association of ACE I/D polymorphism with Obstructive Sleep Apnea Syndrome (OSAS)and OSAS accompanied by hypertension.Methods The PCR-RFLR was used to test the I/D polymorphism of ACE genes of 80 OSAS patients with monitoring their multi-lead sleep map,and 60 health people were selected as control.Results The distribution of the II,ID and DD,ACE genotye was 25%,50%and 25% in the health people and 52%,38% and 10% in OSAS patients,respectively.The differences were significant(P<0 05).The frequencies of D and I allele were 50% in the health people and 29% and 71% in OSAS patients.And also the frequency were significantly different(P<0 05).The frequencies of D and I allele were 36% and 64% in the OSAS patients;22% and 78% in OSAS accompanied by hypertension.The difference were statistically significant(P<0 05).The OSAS patients accompanied by hypertension had more longer average apnea time(P<0 05),more severe AHI(P<0 05),minimum SaO 2(P<0 05)and frequence of SaO 2<90%(P<0 05) than OSAS patients.The OSAS patients with I/I genotype had more severe AHI(P<0 05),longest apnea time(P<0 05),and frequence of SaO <90%(P<0 05) than that in OSAS patients with I/D or D/D genotype.Conclusion There is an association between morbidity of OSAS and I/D polymorphism of ACE gene.Both of II genotype and I allele are risk factors of the morbidity.The OSAS patients with I allele are easily accompanied by hypertension.
