腹腔镜一针缝合法修补十二指肠球部溃疡穿孔

目的探讨腹腔镜一针缝合法修补十二指肠球部溃疡穿孔的可行性。方法总结42例治疗经验，对手术适应讧、操作技巧、相关体会和注意事项等作分析。结果42例均获成功，手术时间80～150min，平均105min，术后服用3个月的质子泵抑制剂，第3周和3个月各检查胃镜，证实渍疡痊愈或疤痕愈合，该组无手术死亡、腹腔残余脓肿、肠瘘、幽门狭窄及梗阻等发生。结论腹腔镜一针缝合法修补十二指肠球部溃疡穿孔，具有操作简单、创伤小、恢复快及住院时间短等优点，值得应用推广。

[ Objective ] To explore the feasibility of needle sewing laparoscopic repair duodenal ulcer ball per- foration. [Methods ] Surgical indications,operating skills,relevant experience and precautions wre analyzed in 42 cases received needle sewing laparoscopic repair duodenal ulcer ball perforation. [ Results ] 42 cases have been suc- cessful, operation time 80~150 min, an average of 110 min.The patients took three to six months of proton pump in- hibitors after operation, gastroscope inspection was conducted week 3 and 3 months later, ulcers healed or scar heal- ing, no operative death, abdominal abscess and residual intestinal fistula, pyloric stenosis and obstruction there. [Conclusion] A needle sewing laparoscopic repair duodenal ulcer ball perforation, with simple operation, small trauma, quick recovery, shorter hospitalization time etc, and is worth popularizing.
