用活体动物进行冠脉搭桥训练的效果分析

摘要 目的:通过在活体猪心上进行冠脉吻合术,寻找快速提高吻合冠脉血管水平的方法。方法:对12名外科医生进行为期12个月的冠状动脉吻合训练,通过冠脉造影检查搭桥术后吻合口通畅及出血情况。于训练的第3、6、12个月进行考评,记录吻合乳内动脉与前降支所需要的时间与血管吻合口质量。结果:培训者完成猪冠状动脉搭桥手术所需时间:训练初期平均(22.08±1.14)min、中期平均(17±1.14)min,末期平均(13.16±1.14)min,组间相比差异有统计学意义(P<0.001);训练末期血管吻合口质量显著高于训练初期(P=0.001)。结论:通过在活体猪心上进行冠脉搭桥训练可快速提高冠脉血管的吻合技术,为心外科医师熟练掌握冠脉搭桥术奠定了理论基础与实验基础。 Objective:To systematically train coronary artery bypass grafting in vivo porcine in order to find a quick method of improving skill of coronary artery anastomosis. Methods : Twelve surgeons were trained for coronary artery bypass grafting in vivo porcine within 12 months, and quality of vascular anastomosis and hemorrhage was detected by coronary arteriography. All surgeons were exam- ined in 3rd,6th and 12th months,anastomoses time of arteria mammaria interna and anterior descending branch was recorded,and vas- cular anastomosis quality was also included. Results: The time of coronary artery bypass grafting was (22.08 ± 1. 14) rain in 3rd month, ( 17 ± 1.14) rain in 6th month and ( 13.16 ± 1.14) rain in 12th month,and there was significant difference among three groups (P 〈 0. 001 ). The quality of vascular anastomosis in 12th month was significantly superior than that in 3rd month (P = 0.001 ). Con- clusions : Systemic training of coronary artery bypass grafting in vivo porcine may quickly and significantly improve coronary vascular an- astomosis technique ,which can lay the theoretical and experimental basis for cardiac surgeons.
