不稳定性骨盆骨折的临床手术方法分析

Clinical Analysis of Operation Method of Unstable Pelvic Fracture

目的:探讨不稳定性骨盆骨折的临床手术治疗方法。方法:选取本院骨科在2008年1月-2010年12月收治的56例不稳定性骨盆骨折患者,对所有患者均行切开复位内固定的方法治疗,对B1型骨折患者行骶髂关节前方入路,应用钢板固定;对B2、B3、C1、C2型患者行骶髂关节后方入路,应用加压螺钉固定;对C3型患者行双侧连成一体的骶髂关节前方入路,应用钢板加空心钉固定。结果:本组56例患者,手术时间平均为140 min,术中出血量平均为560 mL;根据Matta判定标准,优秀34例,良好17例,尚可3例,差2例,优良率为91.1%。结论:对于不稳定性骨盆骨折患者应根据患者的具体情况选择合适的入路和固定方式,以使患者尽可能地进行早期活动,促进骨折的愈合,提高治愈率。

Objective：To investigate the clinical operation method of treatment of pelvic fracture instability.Method：Fifty-six patients with unstable pelvic fracture in our hospital from January 2008 to December 2010 in Department of orthopedics were selected,all patients were treated by open reduction and internal fixation methods,type B1 fractures were treated with sacroiliac joint approach,application of plate fixation;on B2,B3,C1, C2 in patients with sacroiliac joint posterior approach,application of compression screw fixation;joint in front of C3 patients underwent bilateral fused sacroiliac approach,application of plate and cannulated screw fixation.Result：Of the 56 patients,the mean operation time was 140 min,the amount of intraoperative bleeding was 560 mL;according to the criteria of Matta,excellent in 34 cases,good in 17 cases,3 cases were fair,2 cases were poor, the excellent and good rate was 91.1%.Conclusion：For patients with unstable should choose the appropriate approach and fixation methods according to the specific circumstances of patients with pelvic fractures in patients with as much as possible,in order to make early functional exercise,promote fracture healing,improve the cure rate.
