摘要
目的 探讨CT对于泪腺区肿瘤性病变的诊断价值.方法 对36例(45眼)泪腺区肿瘤性病变的CT表现进行回顾分析.结果 36例(45眼)中男17例(19眼),女19例(26眼).年龄15~81岁,平均39岁.右眼16例,左眼11例,双眼9例.CT表现:病变呈不规则形15例,长圆形14例,扁平形7例,卵圆形5例,分叶状4例;21例边界清楚;12例密度不均匀,其内可见低密度区和(或)钙化;7例沿眶外壁向眶尖区生长,与外直肌分界不清;邻近眶壁骨质虫蚀样破坏3例,明显溶骨性破坏1例.除3例(6眼)双眼泪腺炎性假瘤患者接受药物治疗外,余均经手术治疗.其中泪腺炎性假瘤11例(15眼),泪腺多形性腺瘤14例(14眼),其中包括2例(2眼)复发病例,泪腺恶性多形性腺瘤2例(2眼),泪腺腺样囊性癌2例(2眼),泪腺腺癌1例(1眼),泪腺导管囊肿2例(3眼),米枯利兹病1例(2眼).结论 CT对于泪腺区肿瘤性病变的定位、定性具有一定的诊断价值.
Objective To investigate the diagnostic value of CT in neoplastic lesion in the region of lacrimal gland. Methods Ret- rospective analysed the CT findings of 36 cases (45 eyes) in the region of lacrimal gland. Results There were 17 cases of male patients ( 19 eyes) and 19 cases of female patients(26 eyes). The age ranged from 15 years to 81 years,mean age was 39 years. There were 16 cases in the right side,ll cases in the left side and 9 eyes in bilateral,CT findings: 15 cases with irregular shape,14 cases with oval shape ,7 cases with flat shape ,5 cases mimic orbicular- ovate ,4 cases with lobulated shape. 21 cases has clear margin, 12 cases with heterogeneous density in which contain low density and(or) calcification. 7 cases grown spread from lateral wall to orbit apex and was demarcation indistinct with lateral rectus muscle,3 cases has worm -eaten appearance of orbital sclerotin, lcase manifest osteolytic de- struction. All patients underwent surgical treatment except 3 cases of lacrimal gland inflammatory pseudotumor which included 11 cases (15 eyes) of lacrimal gland inflammatory pseudotumor, 14 case( 14 eyes) of pleomorphic adenoma which relapsed in two cases ,2 ca- ses (2 eyes) lacrimal gland malignant pleomorphic adenoma,2 cases of lacrimal gland Adenoid Cystic Carcinoma, 1 cases of adenocar- cinoma of lacrimal gland,2 cases of lacrimal duck cyst, and a case(2 eyes)of Mikulitz disease. Conclusion CT has certain diagnostic value in the location,qualitation of the neoplastic lesion in the region of lacrimal gland.
