生物补片在盆底重建手术中的应用 被引量:6

摘要 女性盆底功能障碍性疾病(pelvic floor dysfuntion,PFD)主要包括盆底器官脱垂(pelvic organ prolapse,POP)和压力性尿失禁(stress urinary incontinence,SUI),是中老年女性的常见病、多发病,严重影响着中老年女性的生活质量.1%的患者需要通过手术来纠正POP和SUI,其中29%患者需要再次手术[1].传统的盆底重建手术采用自身薄弱的组织和韧带进行加固,常导致POP和SUI复发.为提高治疗效果,人工合成网片和生物补片广泛用于修复重建盆底组织.随着盆底整体理论、吊床学说及女性盆底支持结构3个水平理论的确立,手术器械的改进以及植入材料的发明应用,盆底重建手术的目标不仅是修复受损的组织,而是在修复的基础上通过提供各种形式的支持物使组织替代和再生.人工合成的不可吸收补片材料坚固,应用方便,但可能发生感染、排斥和侵蚀等并发症,生物相容性较差,导致更复杂的外科问题,需要更高的治疗费用.
