血清C-反应蛋白水平与妊娠期糖尿病的关系

THE RELATIONSHIP BETWEEN SERUM C-REACTIVE PROTEIN LEVEL AND GESTATIONAL DIA-BETES MELLITUS

目的探讨血清C-反应蛋白(CRP)水平与妊娠期糖尿病之间的相关性,为妊娠期糖尿病的早期防治提供依据。方法选取2012年4月—2013年4月在我院行产检分娩并确诊为妊娠期糖尿病的132例孕妇作为观察组;选取同期就诊、孕周及孕次相同的正常孕妇72例作为对照组。检测并对比二组孕妇的孕龄、孕前BMI、空腹血糖(FBG)、胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)、血清胰岛素(FINS)及CRP水平,应用多因素Logistic回归模型分析各项指标与妊娠期糖尿病的关系。结果观察组的孕龄、孕前BMI、CRP、FBG、FINS、TC、TG和LDL-C均明显高于对照组,差异具有统计学意义(P<0.05)。多因素Logistic回归模型显示,CRP、FBG、TC、FINS水平是妊娠期糖尿病的危险因素(OR=12.552,P<0.01);血清CRP水平≥4.80mg/L是妊娠期糖尿病的独立危险因素。结论血清C-反应蛋白水平是妊娠期糖尿病的发生、发展的重要指标之一。

Objective To explore the relationship between serum C - reactive protein（CRP） level and gestational diabetes mellitus and to provide evidence for early prevention and treatment of the disease. Methods Totally 132 cases of pregnant women with gestational diabetes mellitus were selected as a observation group from April 2012 and April 2013, other 72 cases of pregnant women with the same gestational weeks and gravidity were selected as control group in the same period. The gestation age, progestation BMI, fasting blood glucose （FBG）, cholesterol （TC）, triglyceride （TG）, low density lipoprotein（LDL - C）, high density lipoprotein（HDL - C）, serum insulin （FINS） and CRP levels of the two groups of pregnant women were detected and compared. Multiariable Logistic regression model was to analyze the relationship between each index and gestational diabetes mellitus. Results The gestation age, progestation BMI, CRP, FBG, FINS, TC, TG, LDL - C level of the observation group were significantly higher than that of the control group, the difference had statistical significance （P〈0.05）. Multiariable Logistic regression model showed that the CRP, FBG, TC, FINS level were the risk factors for gestational diabetes mellitus; serum CRP≥4. 80mg/L was the independent risk factor for gestational diabetes mellitus. Conclusion The serum level of C - reactive protein is the important indicators of occurrence and development of gestational diabetes mellitus.
